CTK EDGE Parent Consent Forms

If you wish to attend any of the Edge events that will be held away from CTK Church property, you will need to give us a Parent Consent Form for each event.  Please print and fill out the forms below: 

Bathing suits for guys and girls should be modest. If you have any question about whether it is modest enough, you should not wear it. Girls must wear a modest one-piece bathing suit with shorts over. Bikinis and triangle tops are not allowed- girls' bathing suits cannot make any kind of "v" in the chest. Bathing suits should have no cutouts where skin is showing. If you do not have a bathing suit that fits into these criteria, you can wear a colored t-shirt and shorts over your own bathing suit. Be prepared for our chaperones to ask you to wear a t-shirt over your bathing suit if they agree that it is not appropriate.
Boys' boxers should not be seen under too short bathing suits (or shorts).
